School nurses are responsible for first aid care of children who are injured or become ill while under school supervision. In such an emergency, every effort is made to contact the family. The success of this procedure is dependent upon accurate information from parents. An emergency information card/form is sent home in September from each school and should be returned immediately to the appropriate school office, classroom teacher, or homeroom teacher. If your contact information changes during the school year, please notify  Debbie Robinson, District Registrar to update your information.  657-6121 ext. 4004 


Athletic Program: Students are required to have a current physical (within 12 months) on file in order to participate in interscholastic sports. Parents also need to complete the Medical Recertification/Parent Permission form prior to each season, to determine the physical capability of a student to participate. Recertification and parent forms are now all ONLINE on the school Athletic page. (If there is a family who is unable to access the internet to complete the forms please contact the Athletic Director; Jon Mastin for assistance.) A student needs one physical for all sports, unless an injury or an extended absence occurs during the year. If a student is absent from school for five or more consecutive days or receives an injury, they must obtain clearance from their own physician and approval from the school physician before returning to competition. All paperwork needs to be filed with the school nurse before the start of the first practice.
