Phyiscal Examinations & School Health Screening

Physical examinations are mandated for students entering grades UPK, K, 2, 4, 7 and 10. Parents are urged to have their family physician administer such physicals at these levels. If a physical is not done by the family physician, the school physician is available to conduct an examination here at school. At this time, it is also a good idea to have a complete dental examination done by your dentist. Mandated school physicals must be done after September 1 of the previous school year.

You may download our Physical Examination form from this website (see file below). This form should be given to your health care provider and returned to the nurse's office.


All students are screened for weight, height, vision and hearing each year. Additionally, 5th grade students are screened for scoliosis.

*Mandatory State Audits or Surveys*

The NYS Department of Health (DOH) does periodic random audits of immunization records that are already available with identifying information on the NYS Immunization Registry. However, if you do not wish your child's name to be on an immunization record during these mandated reviews, please advise the school nurse at the start of the school year. The DOH also requires periodic audits of body mass index and weight status categories as reported by your private physician/nurse practitioner with mandated physical exams. These audits are anonymous; however, if you prefer not to participate in these audits, please advise the school nurse at the start of the school
